Maybe he just had some adventures right after he regenerated and didn't bother to properly look in a mirror or anything? If he was kind of in a self-loathing mood and managed to keep really busy visiting disasters, he probably could have gotten by only seeing little glimpses of his reflection for a fairly decent amount of time.
